In Case of Dental Emergency:
Please call the office. The dentist is on pager and will be contacted immediately.

When our office is closed, our 24-hour answering service is always available to take your message. We're closed on weekends and major holidays. And sometimes we close for a short while to update our procedures and skills at a continuing education program. That way, we can always offer you the latest techniques and the benefits or our up-to-date knowledge.

The office telephone number is 925-2118. We operate by appointment only, and ask you to please call ahead of time so that we can be properly available to you. If you're unable to keep an appointment, please let us know right away; preferably, please give us 24 hours' notice. That way, we can give your unused time to another patient, and be fully available for you at your new appointment.

Emergency Care
Sometimes an emergency situation can arise. If you do have an emergency, please call us as early in the day as possible and we will do our best to respond promptly. If the problem arises while the office is closed, our answering service will forward your message to the doctor; or if he is unavailable at that time, to whomever is on call.

Telephone Calls
If you have any questions about dental problems, please call. We ask you to understand however, that the doctor might be busy with another patient when you call. Our family dentistry office staff is very knowledgeable and will be glad to answer your questions. If for any reason you do need to speak to the doctor, he'll return your call as soon as he can.

Recall Visits
We hope you share in our belief that regular preventive dental health care is a sound investment. While the responsibility for returning for this treatment rests primarily with you, we will provide the service of sending you a written reminder when it is time to return. Please telephone for an appointment when you receive this notice.

At the time of your recall visit, please advise the receptionist of any change in the following: (1) address, (2) telephone number (3) health, (4) medication being taken, (5) marital status, (6) employment, and (7) insurance coverage.

Fees and Payments
We make every effort to keep down the cost of your dental care. You can help by paying at the time of your visit. If your treatment program requires several visits, you will be given an estimate and asked to discuss definite financial arrangements with a member of our business office staff. Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment.

Insurance
We follow a program that simplifies the preparation of insurance claim forms, thereby holding down the office costs that are unrelated to the delivery of good dental care. The receipt form we give you at the end of each visit contains all the information needed from the dentist for your insurance company to process your claim.

If your insurance company has not furnished you with a claim form, we will be happy to provide you with a universal claim form to which you can attach one copy of the receipt form. You can then fill out your identifying information at the top of the form and forward it to your insurance company.

Please remember that no insurance company attempts to cover all dental costs. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, co-insurance, or any other balance not covered by your insurance company.
